1. Acceptance of Terms
By accessing or using the services of Salita Healing LLC ("Salita Healing," "we," "our," or "us"), you ("Client," "Patient," or "you") agree to be bound by these Terms and Conditions ("Terms"). If you do not agree to these Terms, please do not use our services.
These Terms apply to all services provided by Salita Healing LLC, including but not limited to chiropractic care, functional medicine consultations, Exomind therapy, Emsella treatments, and any other wellness services offered at our facility located at 5000 W 36th St, Suite 120, Minneapolis, MN 55416.
2. About Our Services
Salita Healing LLC is owned and operated by Dr. Carrie Getzmier, a licensed Chiropractor and Functional Medicine Physician. We provide integrative health and wellness services including:
Chiropractic care and spinal adjustments
Functional medicine consultations and treatment plans
Exomind neurostimulation therapy
Emsella pelvic floor treatment
Other wellness and therapeutic services as offered
Our services are provided for general health and wellness purposes. Nothing on our website or in our communications constitutes medical advice, diagnosis, or a doctor-patient relationship unless explicitly established through a formal consultation.
3. Not an Emergency Service
Salita Healing LLC is NOT an emergency medical service. If you are experiencing a medical emergency, call 911 or proceed to the nearest emergency room immediately. Our services are not intended to replace emergency medical care.
4. Appointment Scheduling & Cancellation Policy
4.1 Scheduling: Appointments may be scheduled via our website, by phone at 612-429-6929, or by email at drcarrie@salitahealing.com.
4.2 Cancellations: We require at least 24 hours’ notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee as outlined at the time of booking.
4.3 Late Arrivals: Clients arriving more than 15 minutes late may need to reschedule at our discretion. The full appointment fee may still apply.
4.4 No-Shows: Clients who fail to appear for a scheduled appointment without prior notice may be charged a no-show fee and may be required to prepay for future appointments.
5. Payment Terms
5.1 Fees: All service fees are disclosed prior to treatment. By proceeding with services, you agree to pay the applicable fees.
5.2 Insurance: Salita Healing may or may not accept your insurance plan. It is your responsibility to verify coverage with your insurance provider. You are responsible for any amounts not covered by insurance.
5.3 Payment Methods: We accept payment by credit card, debit card, cash, and other methods as specified at our facility.
5.4 Refunds: Refunds for prepaid services are handled on a case-by-case basis. Completed services are generally non-refundable.
6. Health Information & Informed Consent
6.1 Accuracy of Information: You agree to provide accurate, current, and complete health information. Salita Healing is not responsible for adverse outcomes resulting from inaccurate or incomplete health information provided by you.
6.2 Informed Consent: Before receiving certain treatments (including Exomind and Emsella), you will be asked to sign an informed consent form acknowledging the nature of the treatment, potential risks, and alternatives.
6.3 Contraindications: Certain treatments may not be appropriate for all individuals. You agree to disclose any medical conditions, implants, medications, or other relevant information that may affect your eligibility for treatment.
7. Exomind & Emsella Specific Terms
7.1 Device-Based Therapies: Exomind and Emsella are device-based therapeutic technologies. Results may vary and are not guaranteed.
7.2 Temporary Location: Please note that our Exomind and Emsella services are currently provided at 5000 W 36th St, Suite 120, Minneapolis, MN 55416. This location is temporary and subject to change through October or November. We will notify you of any address changes in advance.
7.3 Eligibility: These therapies are not suitable for individuals with certain medical conditions including, but not limited to, active implants (e.g., pacemakers), pregnancy, or active cancer. Full contraindication lists will be reviewed at the time of consultation.
8. Intellectual Property
All content on our website (salitahealing.com), including text, graphics, logos, and images, is the property of Salita Healing LLC or its content suppliers and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written permission.
9. Limitation of Liability
To the maximum extent permitted by law, Salita Healing LLC, its owners, employees, and agents shall not be liable for any indirect, incidental, special, or consequential damages arising from or related to your use of our services or website, even if we have been advised of the possibility of such damages.
Our total liability to you for any claim arising out of these Terms or our services shall not exceed the amount you paid for the specific service giving rise to the claim.
10. Indemnification
You agree to indemnify and hold harmless Salita Healing LLC, Dr. Carrie Getzmier, and their respective employees, agents, and contractors from any claims, damages, losses, liabilities, and expenses (including attorneys’ fees) arising from your violation of these Terms or misuse of our services.
11. Governing Law & Dispute Resolution
These Terms shall be governed by and construed in accordance with the laws of the State of Minnesota. Any disputes shall first be attempted to be resolved through good-faith negotiation. If unresolved, disputes shall be subject to binding arbitration in Hennepin County, Minnesota, in accordance with the rules of the American Arbitration Association.
12. Modifications to Terms
Salita Healing LLC reserves the right to modify these Terms at any time. Updated Terms will be posted on our website at salitahealing.com with the revised effective date. Continued use of our services after such changes constitutes your acceptance of the updated Terms.
